In addition to our Prescription Drug Authorization form you will need to provide a copy of your state medical license. The Medical Director or Pharmacist license must match the signature on your Prescription Drug Authorization form.

- State Medical License
- State Board of Pharmacy: License to Distribute Dangerous Drugs
- Medical Licensing Board
